Standing orders: Assess presence and use of standing orders for
CDC recommended vaccines
Patient reminders: Assess presence, use, and efficacy of patient
reminders for CDC recommended vaccines
Vaccine safety: Assess or collect feedback on staff awareness of
and adherence to vaccine safety policies and procedures
External data sources: Assess or collect feedback on staff
awareness and routine use of external sources of immunization
data (local or regional HIEs, immunization registries, and/or
national networks such as CommonWell and Carequality)
Refer: For vaccines that are referred out, ensure staff know how
and where to refer for vaccines that are not administered by the
health center
Conduct training, as needed, to support staff in following the
standards
Process (Workflows)
Standing orders: Review and assess policies and procedures
related to standing orders for CDC recommended vaccines; where
gaps exist, develop plan to introduce standing orders
Patient reminders: Review and assess policies and procedures
related to identifying and sending patient reminders for CDC
recommended vaccines; where gaps exist, develop plan to send
patient reminders
Vaccine safety: Review and assess health center policies and/or
training on vaccine administration, storage and handling, safety
protocols, standard precautions, and adverse reactions
External data sources: Review and assess policies and procedures
to check external data sources for immunizations
Refer: For vaccines that are referred out, review or design a list of
referral partners by vaccine and disseminate the information; for vaccines where a referral partner does not exist, conduct search to identify partners
Refer: Review and address gaps in referral workflows, including
closing the loop and documenting vaccines administered outside the practice
Technology/Informatics
Standing orders: Assess electronic workflow and
modify/streamline (or develop a plan to modify/streamline) one or
more standing orders
Patient reminders: Assess the electronic workflow and modify/
streamline (or develop a plan to modify/streamline) the process to
identify and send reminders for patients due for one or more
vaccines
External data sources: Assess electronic workflow to view and
incorporate external immunization data; develop plan to fill in
gaps if there is no HIE or IIS connection, including exploring costs,
timeline, and effort to establish connections
Refer: Assess electronic workflows to refer patients for vaccines
that are not administered at the health center
