• Administer/Refer Activity: May use the Word Doc template for gathering responses, but please submit your responses via this Microsoft Form Link

• Complete a 10-chart audit for gaps where the Administer/Refer standard would be applied

• Identify a mini-intervention to apply for 1 - 2 weeks to try and close the identified gaps

• Complete a post 10-chart audit to evaluate effectiveness of the mini-intervention trialed

Please submit weekly “small” data on Confluence by May 23
