We have successfully administered 1913 doses of the Covid Vaccine since Jan 1st through our main clinics and through mobile vaccine events in partnership with DPH and local school districts. BACHC has established a strong presence in the community through Outreach Campaigns and partnerships with local CBO’s and state health departments. We continually provide quality healthcare to the underserved population in the area by conducting street outreach and by using a PCMH model to connect individuals with full scope medical and case management treatment. |

These include formal MOU’s and partnerships with: Antelope Valley Health Neighborhood Service Area 1 Leadership Team (SPA1), SPA1 Virtual Clergy Antelope Valley Partners for Health Wraparound Engagement Desert Outreach Smoke-Free Coalition Antelope Valley Homeless Coalition Palmdale Regional Medical Center Antelope Valley Medical Center Antelope Valley College Olive Support Services Department of Public Health South Antelope Valley Emergency Services Dept of Mental Health Children’s Center of the Antelope Valley Penny Lane Grace Resources Hope of the Valley St Vincent De Paul To name a few! Bartz supports and receives support from all of these CBO’s and coalitions to provide wraparound services to the underserved population within our area. |

Create an opportunity for vaccine hesitant individuals to easily seek out consolidated information. Create or provide easy to understand handouts/fact sheets. Provide credentialed training and presentations that CBO’s or trusted “influential messengers” may use to connect with their clients, patients, families, and peers. |
24b. What would you like to share about vaccination opportunities and provider partnerships?
In the are of SPA1 in LA County, vaccination #'s are below the national average. To effectively reach populations of interest, we believe vaccine clinics and education campaigns in partnership with faith-based organizations are critical in building trust within the Latino and African American vaccine hesitant community. Evidence based results through similar partnerships with other CBO’s have proven successful in reaching populations who have generally poor health outcomes as identified through LACHC’s Key Indicators of Health report. |
