Discussion - How can we approach, in year 2, closer implementation of ODH Data Model? Challenges of data collection? Knowledge gaps and barriers in capturing ODH? HCN - Daniel - the HC, La Casa, on this will be going to Epic, there are mapped fields that front desk can get to as part of patient demographics. Has been disseminated across all sites, how to best ask patients. HCs wrote up scripts for staff to use when asking patients. Daniel is looking at ways to merge the data collected prior to the new data. Industry and Occupation were free text, HCN provided a job aid to find mapped codes and they would add standardized field into free text. Epic already has SOC and NAICS codes already, so HCN is activating them. Keeping open one field for free text and standardized occupational, then in a few weeks the free text field will disappear. La Casa has been on Epic for 2 weeks. OCHIN - AnneMarie Overholser Both fields, OCC and IND were added to Epic. Have not heard from HC about success and challenges in using the field, will be running a report. Same concern about having data in free text - OCHIN does not want to lose the valuable data that’s in there, but trying to move it to structured field. Also moving this to several health centers. There is a current Epic upgrade across the board, so there are resourcing constraints and a timeline for implementation of certain features. Will back end support SOC and NAICs codes? AllianceChicago - JEna Wallander Gemkow - AllianceChicago did a training in August with providers, have met with Genny about how data is captured in the tool. The provider champion is enthusiastic about this project and serving a good role model for others. Will be doing data pull. Julia - Workflow - When ODH model was designed, it was thought that the patient would be able to Providers are overwhelmed with quality measures, appreciate the idea of taking it out of the providers. What are most challenges and most successful? HCN Edelise - Would be ideal to have a kiosk where patients can enter data themselves. Will be interested to see if La Casa adopts the kiosk at this time. More and more HCs are using tablets and kiosks. Daniel points out HCs are looking at having both options - kiosk and tablet. Patients may need some education on what the data means, what is used for. Using MyChart could help as well. Conto is used for tablet. NIOSH - There are tips in the Guide that Ray showed.
