2 min | Welcome | Group | Lesson Learned: Leon: Thought that putting a specific SOP in place would be most effective way to share best practices, but more providers need to be reached and team needs to remain engaged with the providers. Instead of SOP, there is now an action plan in place for each provider and an established line of communication with each provider (electronically, including data and performance updates). Need behaviors and results of behaviors included. Focus on two key things and doing them well instead of doing 5 things perfectly Antoinette: Provider engagement is so productive, a trend across all participating health centers Dr. Lawrence: Finding more creative ways to address patients and engage providers Alyssa: Provider engagement is important, because patients don’t have that knowledge and it is on provider to start that conversation |
