C19 CyncHealth Partner QI/Interoperability Challenge

Goals of the Effort: Completeness of COVID patient admission data.  Hospitals are off loading to contracted SNF’s but as hospital level of care.  Want to assure as these patients are not lost out of the data set when they transfer. 

Scope of Effort  (What activities are included and which are out of scope):

In Scope  

  • Assuring ADT connections. 

  • Understanding Level of care movement if stay in place and become SNF or LTC. Inclusive of health center practices and patients.

  • Helps to understand the outcomes of patients that contracted COVID or had post-COVID symptoms.

Stakeholders: Facilities contracted by State of NE to accept these transfers,  State of NE, CyncHealth

Workplan (high level: major steps): 

  1. Onboarding:

    1. Engage NE DHHS to obtain list of contracted facilities

  2. Execution:

    1. Determine if these are existing CyncHealth connections

    2. Does the connection meet our ADT standards

    3. Do rapid ADT connections if needed

  3. Testing and Validation:

    1.  Validate ADT to USCDI standard technically

    2. Validate Clinically for Data Quality

  4. Sustainability:

    1.  Maintenance of any connections initiated during this process

Measures of Success:

No loss of COVID Patient tracking across transfers outside a hospital system

Challenges Anticipated:

  1. Not all SNF locations data sharing with HIE

  2. ADT connection will limit to those data elements versus a connection that includes labs and transcriptions
