Jim has wanted to focus on LA, Houston, Queens, Chicago, Detroit, Atlanta (Dekalb, Fulton)

NACHC suggests start with CHWs in MI, GA

  • Susan to confirm with Rob and give NACHC Top 10 Cities
Set-up

NACHC will need two weeks to get this set up:

  1.  Facilitator training with Susan for CHW facilitator and DON for Albany
  2.  Virtual set up:
    1. If virtual, will name groups by site (Albany 1, Albany 2), and will use names A, B, C, D
  3.  Recording
    1. Ask sites how best to record
    2. Zoom if virtual
    3. Maybe Zoom on phone to record live
  • Sarah to get dates/times for facilitators from Cassie and Ronda (Wanda and Sarah to join)
  • Sarah to create checklist for Coordinators/Facilitators for set up
Expansion

Subgroups that we are NOT thinking about

  • Native health (natural medicine): Austin, Seattle, La Maestro (San Diego)
  • Deep rural (Montana)
  • Pregnant women coming out of detention
  • Homeless
