• Nurse currently seeing patients, so just Ana today

• In the process of changing EMR to Epic, Medical Director at conference at that now - planning for Sep - Oct transition, currently on Green Ways(?)

  • NACHC can connect them with others who have moved to Epic if they need any assistance

  • When we get to Document, we can focus that on a future workflow and what it will look like for them as they transition

• Interested in focusing on Shingrix, need to discuss with Medical Director

  • Still discussing what population of focus will be, first will look at patients with 2+ chronic conditions as a start

• From COVID side, seen a huge decline in patients willing to vaccinate or already have been

• Get their supplies from HRSA

• Pneumo rates also low, want to integrate providers in monthly meetings and discuss how to increase pneumo vaccination rates in the clinics

  • Using chart audits to drive conversation

• Most MAs had a conversation with patients and give out pamphlets, but most didn’t move forward

• Chart audit review

  • For COVID charts, unable to determine whether recommendations were made (0s marked) because they were not documented

    • 1 means assessment and recommendation in the chart

    • Ana will follow up on question of why some patients did not have COVID recommendation but had pneumo rec

    • Problem with staffing & staff retention

      • Triage done by MA, then go on to provider unless patient is a walk in and determined patient can see a nurse

    • Next steps to include all providers in meeting/include Ana in monthly meeting and find vaccine champion

  • Difficulty doing outreach, most patients are experiencing homelessness

• DUA - they have approved the language, Ana Velez will be the one sending data
