  • Brochures/pamphlets - educational materials for flu season

  • Difficulty pulling SDOH

    • Some free text, some structured

    • subpopulations - high risk

      • Certain prevalent SDOH among 4 levels of risk

        • ACG from Hopkins - Care Analyzer (state) & use i2i system defined risk categories

    • Reports for vaccine administration?

      • Built themselves - not difficult in i2i but at a lull because of transformation issues - no structured reports

        • Recommended to use UDS reporting

  • Want to improve vaccination rates for influenza and pneumo

    • Absence of reminder system

    • lack of immunization records (incomplete)

    • CTWIZ - cannot be pulled into the EHR, needs to be manually input

  • Mobile unit, shelter - data is connected to the cloud straight into ECW

    • everyone is a patient who is getting a vaccine - demographic info is recorded

  • Reporting tool? Use ECW Query Studio, i2i for pop management and is interfaced with ECW

    • Reports on regular basis? Area falls short on, tracking immunizations, uses huddle reports for care coordination - could use tools more effectively

  • Do they use PRAPARE? CHWs mainly collect SDOH data, MAs and medical support team could improve on collection

    • Subpopulations - have improvement project on shelter population

  • How much do they trust the data?

    • A lot is missing, they believe the data they have but they know there’s missing info

  • Would like educational materials on vaccine hesitancy in English and Spanish
