Note completed Application Review Forms are to be submitted no later than Thursday, November 30th, 10 am ET.
Each reviewer will have no more than 5 applications to review and score, and each application will be reviewed by three reviewers. A zip file containing all of the relevant documents for each application will be found below.
Maintain the confidentiality of all applications and applicants. Out of respect to the applicants, we request that you do not disclose information to anyone outside the review panel.
Record your scoring. Please use one tab on the reviewer form for each assigned application. Each application will be reviewed for the following domains. Detailed criteria will be included in your template.
