• Follow the instructions below and use the form provided to ensure a smooth, equitable, and efficient process.

• Note completed Application Review Forms are to be submitted no later than Thursday, November 30th, 10 am ET.

• Each reviewer will have no more than 5 applications to review and score. A zip file containing all of the relevant documents will be found below.

Instructions:

1. Maintain the confidentiality of all applications and applicants.  Out of respect to the applicants, we request that you do not disclose information to anyone outside the review panel.

2. Record your scoring. Please use one tab on the reviewer form for each assigned application. Each application will be reviewed for the following domains.

   1. Organizational and Network Capacity (50 points)

   2. LTTB Ambassador Program Recommendations (30 points)

   3. Communication Capacity (10 points)

   4. Budget and Budget Narrative (10 points)

3. Draw upon your experience and expertise to recommend whether to accept, decline, or consider an application.  Indicate your recommendation at the end of your Application Review Form and provide a brief comment to support your choice.  Be prepared to discuss your recommendation with your fellow reviewers at a meeting on Thursday, 11/30 at 11 am ET.  

4. Post completed Application Review Forms below in the File Upload Area by 11/30, 10 am ET.
