Participants energized and empowered to continue to meet the needs of health centers, PCAs, and HCCNs so they are positioned as the premiere primary health care system for the safety net in a value-based reimbursement environment.
💡 (Vision) Current and Future State
Advance NACHC/CAD to meet the needs of health centers moving forward through diversified funding and investment in CAD staff to develop as the future of NACHC through progressive responsibility, building trusted work relationships.
📉 (Mission) Aim
To position health centers, primary care associations and health center controlled networks as the premiere primary health care system for the safety net in a value-based reimbursement environment. Advance NACHC’s 6 Strategic Pillars and CAD priorities.
📸 Objectives
1️⃣ To enhance and build trusted work relationships with each other within CAD.
2️⃣ To share what each CAD team does, successful approaches that could be replicated, and recent products.
3️⃣ To inventory unique skills among the individuals within CAD.
4️⃣ To develop SMART objectives that align to our CAD priorities – we can’t understand if we are doing high quality work – or even the right work, if we have no way to measure what success is.
5️⃣ To identify areas that need standardization/improvement/innovative solutions across CAD and identify workgroups to take action on next steps, e.g., develop structures and processes necessary to implement.
6️⃣ To have fun and celebrate efforts, successes, and each other.
📈 CAD Priorities
1️⃣ Invest in the quality infrastructure through optimization of workflows, health information technology and data aggregation.
2️⃣ Integrate services to improve health outcomes including behavioral, oral, and public health.
3️⃣ Invest in the workforce of the future with a focus on aligning health professional training with the new emerging delivery system and the expanded care team.
4️⃣ Build the informatics infrastructure within NACHC to establish uniform measures (Clinical, Administrative and Cost) and practice guidelines to document in real time health center performance, improvement, and impact.
Additionally, we will diversify funding and invest in CAD staff to train and improve skills/capabilities to achieve greater levels of responsibility and support for NACHC membership and the health center field.
