Close immunization gaps through: 

• Data driven quality improvement tools (assessment, feedback, incentives and coaching)

• Harmonization of adult immunization service delivery with clinical and data workflows 

• Working with care teams, extenders and community partners

New goals:

1. Walking alongside networks and PCAs as they close immunization gaps using data, standards of immunization practices, harmonization and innovative care team, extender and community partner work.

PEDIATRIC WEIGHT MANAGEMENT

• Continue to spearhead legacy teams and cultivate additional best practices

• Develop new measures around pediatric weight

• Introduce concepts of pediatric weight management programs and interventions to a broad audience and go deeper through learning communities
