Task 2 Build care continuum dashboard and provide decision support to users towards identify and closing care gaps.

• Create and execute an implementation plan for the care continuum cascade developed as part of Task 1.

• Test and deploy the dashboard in a pilot at one or more health centers.

• Engage in troubleshooting to understand measures and outcomes.

• Discuss challenges and successes of the approach taken.

• Gather data on patients, users and outcomes of the dashboard and its related CDS tools.

• Document proposed scale and spread of the tools developed to support HCV care.
