NACHC has partnered with CDC and the Health Center Controlled Network AllianceChicago to work with Health Centers in developing a collaborative Care Continuum Hep C Cascade. The goals of our project are to build a population health cascade, which shares data with local health center cascades. Our thinking is that if health centers can use local data to perform quality improvements, HCCNs and health departments could use the population level aspect of the cascade to understand their regional epidemic. See our one-pager attached. We are asking for your qualitative feedback on how your respective departments could work with your partner HCCN to improve the content and usability of the cascade, In other words, what may you need added to the cascade to make it useful for your work? Additionally, what are new perspectives in how an informatics driven approach can improve collaboration between HCCNs, Health Centers and Departments of Health?

Attached is the Scope of Work for this project. Dr. Liesl Hagan is our project lead on the CDC side and can answer further questions, along with me who will be your point person at NACHC with Dr Julia Skapik and Andrea Price. Would you please let us know if you have any feedback or concerns about the SOW?

Note that this project and the hepatitis C care cascades involved are distinct from the CDC Division of Viral Hepatitis care cascade presented for state health department development under PS21-2103 (Integrated Viral Hepatitis Surveillance and Prevention Funding for Health Departments). This project is funded under a different CDC Cooperative Agreement, awarded to NACHC, and these care cascades have been developed separately and involve different care cascade definitions and data sources. Your health department’s decision whether to participate in this project will not impact funding under PS21-2103.
