| open ended opportunity connection and regular contact with families but don’t engage with other community resources to alleviate the burden - after school programs, swim classes, exercise classes - strong engagement Barriers - simplifying registration - familiar face to bridge connection - tour? better cadence of communication - being understanding of busy schedules (know your audience)
| health navigation, parenting classes etc - large range of programs - food pantry, after school programs Logistics - referrals? Partnering to provide enabling and social services gaps - have some but YMCA can offer more - connect patients (smooth, warm hand off) Barriers - logistics - paperwork/legalities (part of the process)
| Long standing relationship - employees/families membership need a site - 3 sites within family radius - rent to Tucson parks and rec They have children optimal for recruitment (50/60 percent are patients - overlap) ongoing family lifestyle - family membership and continue membership - subsidize or fully cover parenting classes? big after school classes Is an FQHC is having a cohort at a YMCA currently? Are they biting off more than they can chew or overpromising? Screening will be needed - better understanding of who needs to be at the table Barriers - provider visit, telehealth dietian, behavior health (only bill for on site classes) - option for telehealth? What about the parents? how do we get them involved

Landscape? | adapt adjust and provide support to get patients to them and defer responsibility think through how to really support health centers and families (different than the rest of the people who come to the YMCA)
| Social services/community support - understanding of demanding schedules when communicating education/support to refer patients - need to get patients using it - depending on providers/staff to make the connection is a tall order
