Create Data Upload Submission

1. Click the link above to upload a data submission file

2. Fill out the required fields.

   1. Data Partner: Verify Your Health Center is selected

   2. Email of Reporter: Enter Your Email Address, we will use this to contact you if we have any questions

   3. Data Extract Timeframe: Select the Data Extract Timeframe - If your data has more than one month select all that apply

   4. Summary: Please specify the project name, partner you are submitting data for, and the date ranges covered. Ex. Adult Immunization_Health Center X_July 2023

   5. Description (OPTIONAL): If you would like to send us any additional notes or feedback, please write them here.

3. Ignore the Assignee and Watcher fields, for internal use only

4. To attach your data to the ticket, drag the file(s) from your file explorer into the ticket body. Alternatively, click 'Browse' and select your file(s) from the popup window.

   1. Data file format (no spaces) (use underscores): 

      1. (YYYY_MM_DD)_(NACHC Project Name)_(Name of Partner).xls     

      2. Example: 2020_04_01_Adult Immunization_ABCPCA.xls

5. Ignore the Field for Sprint, for internal use only.

6. When finished, scroll to the bottom of the form and click 'Create' to finalize your data submission.

7. Check the chart below to review the status of your previous data uploads.
