Chosen intervention: Looking at noncompliant patients to see why a patient is not compliant, finding patients who are not on a statin but should be, having team member reach out to schedule appointment, via telehealth or hybrid visit (with provider via telehealth but support staff visit patient’s home), provider would have a conversation with patient about a statin and prescribe at this visit
