Monthly Reporting with the Center for State, Tribal, Local, and Territorial Support (CSTLTS) Project Officer
Associate Vice President, Kathy McNamara meets monthly with the Project Leads and CAD’s Grants Administration to track progress on projects and identify opportunities.
● Manager, Grants Administration sends Project Leads (via email) a link to the latest CDC Monthly Reports folder in the Clinical Affairs Division section of SharePoint.
o Folder contains a table with columns for each month where Project Leads provide brief updates for the requested time period or month, i.e., July 1 – 31, 2022 or May 1, 2022 – June 30, 2022
● Project Leads are given a deadline to complete their updates: usually 3 days prior to meeting with the CSTLTS Project Officer
● Once the updates for each project are completed, the final table is sent to the Associate Vice President. She reviews and sends final table of updates to CSTLTS.
Annual Reporting and Performance Progress and Monitoring Report (PPMR) for CSTLTS
There are two sections of the PPMR
Section 1: Provide information on the progress of current and past activities.
Section 2: Propose continuation activities for currently funded projects.
The Performance Reporting section of the PPMRs must include the following:
● Project Status and Results: Recipient must report progress on activities and outputs outlined in the Work Plan.
● Performance Measures (including process and outcome measures): Recipients must report on performance measures for each budget period and update measures if needed.
▪ Process measures are used to track the progress of activities being implemented
▪ Outcome measures to track progress toward achieving estimated outcomes.
● Project Challenges
Recipients must also describe challenges that may affect their ability to achieve annual and project-period outcomes, conduct performance measures, or complete the activities in the Work Plan. Recipients must describe any additional challenges (e.g., identified through evaluation results or lessons learned) encountered during the reporting period.
More detailed guidance regarding the completion of PPMRs, including the Recipient Dashboard User Guide, will be provided by CSTLTS in an email to recipients.
CAD’s Reporting Process:
● Grants Administration Team, with the Associate Vice President develop a progress report timeline for CAD staff and Project leads.
● Project Leads are provided a reporting template
o Section I: PPMR – Project Lead is provided a NACHC-developed PPMR Template to report progress (this is a word doc.) in Clinical Affairs Division SharePoint. Deadline provided on when progress reports are due.
▪ Progress Report is reviewed and revised internally, as needed
▪ After review is finalized, progress report updates are copy & pasted into the CDS PPMR report template
Section II. Year 5 Continuation Work Plan

CAD’s Reporting Process:
● Grants Administration Team, with the Associate Vice President develop a progress report timeline for CAD staff and Project leads.
● Project Leads are provided a reporting template
o Section II: Continuation Work Plan – Project Lead is provided the CDC PPMR report template (see attached) to complete for continuation of the project in the Clinical Affairs Division SharePoint.
▪ Completed Continuation Work Plan is reviewed and revised internally
▪ Budgets developed by Chief Medical Officer, Deputy Director, and Associate Vice President in consultation with Project Leads
▪ Completed budgets and PPMR/Continuation Work is submitted to Director of Finance and Chief Financial Officer for their final approval
▪ PPMRs/Continuation Work Plans with required attachments as identified in CDC Recipient Guidelines are uploaded in the CSTLTS Recipient Dashboard
